Nobody likes to talk about health care rationing - and that's a big part of the problem. 

The sad fact is, health care rationing - that is, the withholding of at least some useful medical services from at least some people who would benefit from those services - is an economic imperative. Most health care economists accept the need to ration health care as a first premise, virtually as an axiom.

Policymakers can't even talk about making minor (necessary) changes in Social Security without having their heads handed to them. For them to talk openly about something as monumental as rationing health care would be suicidal.

Since we can't ration openly, the only other alternative is to ration covertly.  Which is precisely what we're doing today. In fact, fundamental to the understanding of our health care system is recognizing how widespread, vigorous, and enthusiastic the covert rationing of our health care really is.
